When this is completed you will be taken to the page below with the following options:

for your situation. If you are a single player who needs to be placed on a team, you will have the option of being placed with a friend and entering their details. If not, then just select No.
If you are already part of a team. Select the "I am already on a team" option and you will be taken to a page where you will enter your division and the team name.
